New to the District Checklist

Prospective New Students

  • Register your child for school using FinalForms
  • Below are the required documents for registration/screening that need to be uploaded into FinalForms prior to your registration/screening appointment
    • Child's birth certificate
    • Child's social security card or number
    • Custody papers, if applicable
    • Immunization records
    • Parent photo ID
    • One proof of residency: mortgage statement, signed lease agreement, bank statement, or current utility bill
  • When this step has been completed, please contact the office to set up an enrollment/screening appointment.

Note: If you are transferring into the district you will need to Sign a Records Release form below.

Preschool applicants also need to complete the following prior to their screening appointment: 

Preschool & Kindergarten applicants must contact the school office once FinalForms is completed to setup a screening time. Screenings take place in the Spring. Be sure to keep an eye on the calendar and our Facebook page for more information.
